Walmart Certification Form. This form must be completed signed and returned to our office prior to your first shift at a Walmart Pharmacy. It certifies that you have read the previous two training documents.

CMEA Training: Walmart mandates that you must be trained and familiar with dispensing CMEA (Combat Methamphetamine Epidemic Act of 2005) controlled over-the- counter drugs.

Timesheets: PLease download and take a copy of the Timesheet with you to your Walmart shift.
